Outside License Numbers
Tennessee does not accept outside state license numbers. Currently the provider state license number is not a required field when submitting. If you are attempting to submit with a non-Tennessee professional license number, attempt to resubmit the record without the professional license number.
Verify Your Data
The PRE04 field requires a properly formatted TN state license number. Ensure that when you are entering your state license number you follow these guidelines
- Should be a string of five or 6 characters (E.g. 000123 or 00231)
- Should not include any leading alpha characters or digits. (E.g. APN000123 would be entered as 000123)
